What is a Prostate/Rectal Ultrasound?

A prostate or rectal ultrasound is a diagnostic test that employs sound waves to examine the prostate gland or rectum.

During the procedure, a small device known as a transducer, approximately the size of a finger, is gently inserted into the rectum. This transducer emits sound waves that bounce off internal organs and structures. Since these sound waves are beyond the range of human hearing, they are picked up by the transducer after reflecting off the tissues, which are then converted into images.

Your healthcare provider may use an additional tool called a Doppler probe with the transducer. This device allows the provider to listen to the sound waves produced by the transducer, which helps them assess the speed and direction of blood flow through vessels. A lack of sound or faint noises could indicate a blockage in the blood flow.

Why Might I Need a Prostate/Rectal Ultrasound?

A prostate/rectal ultrasound can be used to evaluate the size, location, and shape of the prostate gland and surrounding areas. It may help identify signs of cancer or other conditions, often following elevated prostate-specific antigen (PSA) levels detected in a blood test. Additionally, it can assist in staging and monitoring treatment for rectal cancer and assess the rectum for other issues.

This test may also be employed to guide a needle for obtaining a tissue sample (biopsy) or to position radiation seeds for prostate cancer treatment. Your provider might have other reasons for recommending a prostate/rectal ultrasound.

What Are the Risks of a Prostate/Rectal Ultrasound?

Ultrasound procedures do not involve radiation, making them generally safe. Most individuals may experience mild discomfort from the insertion of the transducer.

If you have a latex allergy, inform your healthcare provider, as the probe is covered with latex before insertion.

There may be specific risks based on your health condition, so it’s essential to inform your provider about all your medical history before the test.

Having excessive stool in the rectum may affect the accuracy of the results.

How Do I Prepare for a Prostate/Rectal Ultrasound?

Your healthcare provider will explain the procedure, and you can ask any questions you may have. It might be helpful to write down your concerns and bring a family member or trusted friend to the appointment for support.

You may be required to sign a consent form granting permission for the procedure. Review this form carefully and ask for clarification on anything you don’t understand.

If the ultrasound is part of a biopsy, you might need to stop taking blood-thinning medications, like aspirin, for about a week beforehand.

Generally, you won’t need to fast or take sedation prior to the test. However, a small enema may be administered before the procedure.

Make sure to follow any additional instructions provided by your healthcare provider.

What Happens During a Prostate/Rectal Ultrasound?

A prostate/rectal ultrasound can be performed on an outpatient basis or during a hospital stay, depending on your situation and your provider's protocols.

Typically, the procedure follows these steps:

  • You will be asked to remove any clothing, jewellery, or objects that could interfere with the exam. If required to change, a gown will be provided.
  • You will lie on your left side on the exam table, with your knees drawn up to your chest.
  • A digital rectal exam may be performed by the healthcare provider prior to the ultrasound.
  • The provider will apply a clear gel to the transducer before inserting it into the rectum, where you may feel a sensation of fullness.
  • The provider will rotate the transducer slightly to capture images of different parts of the prostate and surrounding structures.
  • If assessing blood flow, you may hear a “whoosh” sound from the Doppler probe.

After the test is completed, the provider will wipe off the gel.

While a prostate/rectal ultrasound can be uncomfortable, it’s important to remain still during the procedure. The gel will feel cool and wet. The technologist will make every effort to ensure your comfort and will perform the scan as quickly as possible to minimise discomfort.
